Tsukishima is very tall and has an incredibly thin yet muscular build. Despite being a first-year high school student, he is the tallest player on the volleyball team, standing at 190.1 cm (6' 11") tall. He has short blond hair, pale skin, thin eyebrows, and golden-brown eyes. He is often seen with a frown or a condescending smile on his face.

After joining the Karasuno volleyball team, Tsukishima wears the standard Karasuno volleyball uniform (a black, orange, and white uniform with the number 11 on the back) during matches. He also wears knee pads and white volleyball shoes with green accents. He is also the only player on the Karasuno team who wears glasses. A couple of months ago, Tsukishima began wearing prescription sports glasses given to him by his brother. Outside of games and practices, Tsukishima wears a black gakuran, the Karasuno boys' uniform. He is usually seen with white headphones around his neck. The brand of these headphones appears to be Sony. He has been seen wearing several T-shirts with crescent moons, a reference to his name "Tsukishima," where tsuki means "moon."

Tsukishima is very outspoken and sometimes irritates his teammates due to his antagonistic nature (most notably Hinata, Kageyama, Nishinoya and Tanaka) and sometimes other teams due to his smug and casual attitude, taking pride and enjoyment when his opponents view him as a nuisance as seen with Tendō and Shirabu.

Whenever someone does or says something stupid or foolish, he is seen chuckling. However, he seems to be intimidated by his superiors, and is not seen when Daichi berates Tanaka for antagonizing Aoba Johsai despite the fact that he also did it, or when he backs down when Ennoshita reprimands him. Tsukishima remains calm most of the time, but he can be seen visibly upset.

Tsukishima's family consists primarily of his older brother, Akiteru Tsukishima and his friend and teammate, Tadashi Yamaguchi. His parents are also mentioned, though they don't appear frequently. Akiteru Tsukishima: He is Kei's older brother and used to be a volleyball player. In the past, his relationship with Kei was close, but it deteriorated when Akiteru lied to him about his volleyball performance. After talking about their past at summer camp, the brothers reconcile and begin to rebuild their relationship. Tadashi Yamaguchi: Although not blood-related, Yamaguchi is Kei's closest friend, and their relationship is very close, almost like that of brothers. Yamaguchi has admired Kei ever since Kei protected him from bullies when they were children, and he has been a constant source of support for him, though he also sometimes challenges and motivates him to improve.

Tsukishima is a first-year student at Karasuno High School. He currently plays as one of the starting middle blockers on the boys' volleyball team.
