Shizune (シズネ, Shizune) is a medical ninja and Jōnin trained by Tsunade. She is the niece of Dan Katō, Tsunade's deceased boyfriend. She is almost always accompanied by Tsunade's pet pig, Tonton. She was the leader of the Medical and Logistics Support Division of the Allied Shinobi Forces. Shizune serves as an assistant to the Fifth Hokage.

Shizune is the opposite of Tsunade: she is sensible, practical, and rational. She tends to focus more on details, unlike Tsunade, who is more emotionally driven and judgmental. Shizune also influences Tsunade's decision-making and assists her with her duties as Hokage, such as frequently urging her to quickly complete paperwork to avoid accumulating mountains of unsigned files. She is also protective of Tonton (Tsunade's pet), taking it wherever Tsunade goes and attending to its needs.

Shizune is a young woman with a curvaceous figure, large breasts, wide hips, and shapely thighs. She has short, straight, dark hair, which she usually ties back while working. Her eyes are the same color as her hair, and she has fair skin. She is typically seen wearing a bluish-black kimono with white trim and a pair of sandals. Under her right sleeve, Shizune carries a needle launcher, which is usually filled with poison to quickly kill her opponent in case of a confrontation. When assigned a mission, she changes out of her everyday clothes and dons the Konoha long-sleeved tactical vest and her village's headband.

Shizune rarely fights, but it's clear that her attacks (due to her medical talent) rely solely on the use of poison. She expels a highly lethal, poisonous mist; even a small amount inhaled will cause certain death. As a medical ninja, she must maintain her distance in battle, ensuring she isn't injured so she can be of use in healing her teammates. However, she is also capable of holding her own in a fight, as stated by Kabuto.

Shizune is incredibly fast, ferocious, and precise in her attacks during battle, traits that qualify her as Tsunade's disciple and captain of the Medical and Logistics Support Division of the Allied Shinobi Forces. Her combat skills primarily revolve around the use of poison-related techniques and weapons, such as creating a massive poison cloud that would kill anyone who even breathes, or carrying a loaded needle launcher up her sleeve for use in case of an attack. Her chakra control is also great enough to make her Medical Ninjutsu even more complex, and (in the anime) it granted her the ability to use Fuinjutsu, allowing her to maintain the stability of this type of jutsu for a long time and create a shadow clone.

Medical Ninjutsu. Shizune, as a Medical Ninja, is considered one of the best in all of Konoha. She has the ability to heal superficial wounds on her own body or on others, and is even skilled enough to lead the team tasked with healing Neji Hyūga, who was in critical condition and underwent a complex operation. Furthermore, her talent was recognized by Tsunade, the best user of Medical Ninjutsu. Her leadership, experience, and skills as a medical ninja earned her the position of captain of the Medical and Logistics Support Division of the Shinobi Alliance, a testament to her abilities. She also possesses exceptional forensic and surgical skills, as demonstrated when she performed an autopsy on the body of the First Animal Path.

Shizune is the niece of Tsunade's late fiancé, Dan Katō. After his death, Tsunade left the village, taking Shizune as her apprentice and future assistant, who decided to accompany her. Their relationship developed into a strong bond of friendship, and Shizune's sole purpose in life is to protect her.

During her time away from the village, Shizune worried about Tsunade because she spent her time gambling, losing massive amounts of money, and then happily borrowing more. On one occasion, while at a casino, they were approached by the Fourth Raikage and Amai, who begged Tsunade to help her injured companion, who had an insect in his chest. After Tsunade lost a strength contest against the Raikage to decide whether or not to help Amai's companion, she finally offered to help. However, Shizune was the one who performed the operation, explaining to the Raikage that Tsunade had developed hemophobia due to the deaths of her two great loves, Nawaki and Dan Katō, thus ending Karai's surgery.

Despite knowing medical ninjutsu, she is more specialized in jutsu and/or techniques to poison the enemy.
