Tearing himself away from the bathroom mirror, he picked her up and carried her into the living room. Makarov had recently begun using drugs, which had become his addiction. He laid her down on the sofa and sat down next to her, wanting to rest his head on her lap like before, but as soon as he lay down, he felt a pain in his head. Opening his eyes, he cursed in Russian and was about to speak, but why had she gotten up when he noticed she was gone... simply gone. Nowhere. He hadn't heard her leave, not a trace. His heart pounded, he looked around the room, lowering his eyes to the coffee table. He noticed scattered carpets, which made him rise from the sofa, swaying slightly, approach the table, and inhale deeply again. Closing his eyes, he opened them again and felt the warmth again. Looking down, he noticed her again next to him... a heart-splitting pain immediately reached him; it was an illusion, a hallucination. He couldn't believe it.

After the ultranationalists won the Russian Civil War, Makarov assumed a prominent role in the country's new ultranationalist regime, alongside the newly elected president, Boris Vorshevsky. However, Vorshevsky and his other moderate supporters were aware of Makarov's true plans. They sought to expel him from the party and win the support of the Russian people. Makarov, however, had a tense relationship with the United States and espoused a return to autocracy, the era of the tsars. His visions angered most party members, including Vorshevsky, because they violated the new president's core position. To ensure that the RUNRP had a legitimate representative in the world, Vorshevsky, backed by the majority of party members, expelled Makarov and immediately began attempting to improve relations with the United States. Makarov vowed to eliminate Vorshevsky and his closest associates, calling on his supporters to leave with him. News from his "safe house" confirmed this newfound hatred, explaining the Red Special Forces' continuous attacks on Russia and the West for the next five years.

In 2015, he became the CIA's top target (this can be seen in the Returned to Sender mission briefing).

Early career:

According to intelligence gathered by General Shepard, Vladimir Makarov graduated from the Frunze Military Academy with the rank of captain. He served in the Airborne Forces. Later, during his "career" in the special forces, he served in Chechnya with the rank of captain. His team is believed to have been one of the "toughest" during the campaign.

At one point, the UN conducted an investigation into "human rights violations." Vladimir Makarov was at the top of the list. But the investigation was closed when he decided to leave the armed forces. This was attributed to his pathological hatred of the West and the Russian government. Afterward, Makarov received military training in numerous terrorist organizations: human trafficking, money laundering, bombing military and civilian targets, assassinations, and so on. During this time, Makarov caught the attention of Imran Zakhaev, the leader of a growing "ultranationalist party." This allowed him to be enlisted in the anti-Western movement. Makarov and Zakhaev became partners.

In 1996, Makarov, along with Yuri and other soldiers, were in Pripyat when Zakhaev was involved in a deal to sell fuel rods for gold. He unwittingly witnessed an assassination attempt on Zakhaev, resulting in Lieutenant Price severing his left arm. Makarov saved Zakhaev's life and earned him even greater respect and trust.
