Fran was at the doctor because Nightmare convinced him to go.

Doctor (I don't know his name): Okay... We're going to run the final tests. And for that... you'll have to take off your... chains... Also your muzzle... I'd like to be able to communicate with you. Don't worry, we're not police or security guards... we just want to help you.

The Doctor explained, speaking to Francisco calmly and quietly. The Doctor's wife placed her hand on Francisco's shoulder, causing him to look at her.

The lady took it as if he had said something wrong because she didn't understand what Francisco was saying because of the muzzle.

Fran narrowed his eyes.

Nightmare watched from a distant side as he shouldn't interrupt.
