Kokushibo was a Demon who maintained his human features despite having devoured countless people for 400 years. He maintained a number of rather unusual characteristics: he was a tall man, with a pale skin tone and an athletic and muscular build; his hair was long and black with burgundy edges[6], held by a long ponytail that reached almost to the height of his lower back[5].

On his face, he possessed a series of flame-like markings covering the left side of his head and the right side of his neck; these markings were, in his time as a human, his Hunter's Mark. Kokushibo's most emblematic feature, and at the same time one of his most sinister identifying marks, were his three pairs of eyebrow-less eyes[5] which had a yellowish tint to their irises and a red sclera divided into segments[6]. In the center eyes, his numbering as the Upper Moon One is located, identified with the kanji for First Crescent (壱上弦,, Ichi Jōgen?) indicating his dominant position within the Upper Moons.

As for his attire, he wore a kimono with a violet and black pattern with a white haneri (undergarment worn in a Kimono), a black hakama, and a white obi belt. He possessed a katana made from his own flesh and bones, which was completely decorated with eyes on the cutting blade, the Tsuba, and the Tsuka[7][8]. The guard was also covered in flesh [5] and eyes to fully increase his degree of peripheral vision.

Kokushibo was an extremely strong swordsman, largely considered the second strongest Demon in the series, only surpassed by Muzan Kibutsuji, and holding a high ranking position within the Twelve Demon Moons. Being one of the oldest Demons, his combat experience is overwhelming. Having killed many pillars over the centuries has allowed him to develop extremely efficient fighting techniques to combat Demon Slayers. His extensive experience allows him to face two highly experienced pillars and easily get on their level, easily deciphering their abilities despite only facing them once. Being a user of breath-based techniques, he possesses great levels of strength and reflexes that are enhanced thanks to his physiology as a Demon.

With these qualities he was able to face on equal terms a pillar of the caliber of Gyomei Himejima who was considered the strongest pillar of his generation [1]. Even when Gyomei and Sanemi awakened their hunter marks that increase their physical capabilities beyond their limits [2], Kokushibo had no problems getting to their level and easily surpassing them [3].

In addition to his own Breath of the Moon, as a Demon, Kokushibo can also use Demonic Blood Techniques. In his specific case, he can generate blades of different sizes that resemble growing moons that emerge with each attack [4]. It is believed that the blades are created from their own blood, varying mainly in their coloration which varies from a yellow tone to a brilliant blue [5][6]. The size of the blades can vary as their length and speed making their attacks totally difficult to dodge as well as possessing a very wide range allowing them to corral their opponents with a lot of ease making them practically unpredictable and without an established pattern [7]. This Demonic Blood Technique makes its attacks more powerful, turning each cut made into an extremely deadly one for its victims. Apparently, Kokushibo can also make use of these blades without having to unsheath his katana or perform any cutting movement [8]. The volatile nature of this Demonic Blood Technique makes even the most experienced Demon Hunters have serious difficulties trying to avoid this attack, which stands out at the sight when Sanemi declares that he would not have been able to defend himself from these attacks [9] Kokushibo's Demonic Blood Technique seems to have a secondary ability that allows him to manipulate the shape and reach of his sword's cuts while unleashing his Moon Breath techniques. Its cuts generally create and are characterized by a red/orange crescent shape that bears its crescent moon blades [10]. Kokushibo seems to be able to control these cuts to a certain extent, increasing his scope and form to attack his objective of impossible forms in normal circumstances.

Kokushibo is very demanding of control, but can let his partner take control by riding him. Only if he wants to.
