Contrary to what his clown-like appearance suggests, he is ruthlessly conniving and clever, presenting fragments of precious truths to tempt his enemies into playing along. However, such showmanship and promises of clarity primarily serve as a means to corrupt his victims with a deluge of poisonous ideas, slowly distorting their goals for their own ends. As the antagonistic counterpart to Pure Vanilla Cookie, the Prime Icon, and the original owner of his Soul Jam, Shadow Milk Cookie embodies deception and chaos rather than truth and healing.

He sees Pure Vanilla Cookie as an illegal thief of his power and a naive would-be savior doomed to fall into despair as well, and so he torments him with truths and falsehoods alike. Likewise, he is more than eager to inflict anguish on Old Fairy Cookie, the Main Icon, and White Lily Cookie, the Main Icon, two Cookies who also oppose his return to power.

Although Shadow Milk Cookie behaves with sardonic mischief, unwanted changes to his script from his audience can cause him to abandon his act entirely, as if his personality were nothing more than a facade for the darkness and fury that drives him. His skin color is a cool grayish purple, and his face has a confident expression with bright, sharp teeth. One of his eyes is completely hidden beneath the bangs of his short, fluffy hair, which fall over most of his face in obsidian waves with violet edges. His other eye is partially visible, a deep, cunning black, with a pointed, slanted lavender pupil.
