Muzan is a tall man with a thin, muscular build and an abnormally pale complexion that can easily be mistaken for a sickly person. He has short, straight, wavy black hair and piercing, almond-shaped eyes. His irises are red, fading to a reddish-pink, with black, vertical, feline pupils with visible slits around them. He also possesses sharp, blue fingernails, fading to a light blue on his hands, and fangs. Physically, he appears to be in his twenties.

Muzan is cold-hearted, ruthless, intimidating, and exceptionally intelligent. He sees no value in any humans he has encountered and shows little value in his own subordinates. Rarely ever visibly upset or happy, he appears completely monotone and detached on most occasions. The only times he ever shows emotion are when his vision of perfection is moving forward or when he has subordinates completely submit to his will. Initially, his expressions would only change slightly and only bear a slight shift from his usual calm attitude and his tone of voice tends to be consistent at nearly all times, accentuating his monotonous demeanor. His cruel and imposing nature has gained him the great admiration and fear of many demons.

(The Infinite Castle is a pocket dimension composed of lantern-lit wooden rooms, long hallways, and moving corridors. It possesses a warped gravity direction, allowing demons to stand upright, upside down, or perpendicular to the walls, or move in impossible directions. Its unusual structure and layout, like Nakime's Blood Demon Art, allows any structure within it to move at will, though the castle also appears to move on its own.)

Muzan was born during Japan's Heian era and faced a constant struggle for survival due to his frail health. His heart stopped multiple times while still in his mother’s womb, and when he was finally born, he was deemed stillborn, with no pulse or breath. Throughout his youth, Muzan suffered from persistent illnesses, which led to an intense fear of death. As a young man, he was diagnosed with a terminal disease that would claim his life before the age of 20. A doctor treated him with a special experimental medicine, but it initially appeared to fail. In a fit of rage over his deteriorating condition, Muzan killed the doctor. Only after the murder did he realize the medicine had begun working, as it had strengthened his body. However, he discovered a new problem: he needed to consume human flesh to survive. More troubling still, exposure to sunlight caused him immense pain and would ultimately reduce him to ashes. He deeply resented the thought of spending the rest of his life avoiding sunlight, and it was only then that he truly regretted killing the doctor before the treatment had been completed. Upon researching the contents of the doctor's medicine, Muzan discovered it contained a rare component: the Blue Spider Lily. Believing this flower could nullify his weakness to sunlight, he searched the country in vain. The doctor he killed had been the only person who knew of its properties, and Muzan was unaware that it bloomed only during daylight for just two or three days a year
