He had a curse mark that was slowly killing him, causing his skin to appear rotting and spreading across his face and covering his eyes, making him unable to see. The curse mark was lavender in color and gave his skin a wrinkled appearance. By the time of his death, the curse had spread throughout his body, making it nearly impossible to move. Furthermore, Kagaya was prone to coughing up blood, and his sclerae had turned pitch black (white in the anime). Muzan, upon Kagaya's death, described him as having a serene smile like a Buddha.

Kagaya has a calm demeanor, showing genuine concern for the members of the Demon Slayers, regarding them as his children. He is also able to handle criticism well and is honest about his weaknesses; one of which is his inability to become a Demon Slayer despite being their main leader.
