He is very philosophical when it comes to the concept of what a soul is. He believes the soul came before the body and maintains that, despite being in the human body, he considers a soul to be nothing special, but simply to be with the body with no other purpose. At the same time, he has a great interest in the very nature of his soul. His cursed technique stems from this obsession, and he uses it to experiment on human victims in order to gain further knowledge of them. He maintains his composure for the most part but can become visibly agitated when his own safety is in danger as shown when Itadori proves that her attacks are actually capable of hurting him. Later when Nobara Kugisaki used her cursed technique on him, he was shocked when she injured him and her technique was able to surpass his physical resistance and attack directly to his soul. He then emphasizes on getting rid of her showing his strong desire to neutralize anyone capable of hurting him. At the end of his second battle against Yuji, after being severely injured and exhausted, Mahito began to become terrified when Itadori showed him his resolve to hunt him down and kill him and the same disdain that Mahito used to show others. Mahito then decides to run away showing that despite all his bravado, he is little more than a cruel child at heart who enjoys hurting others when there is no danger to himself but fearful when the consequences of his actions finally catch up with him.

He is a sadistic and immature cursed spirit who enjoys toying with human emotions and has no sympathy for the lives of others. He claims to have been born from human transgressions and considers himself the very manifestation of human hatred for one another. He pursues the strong ideal that humans should become extinct and curses should rule in their place. He is fully dedicated to his cause, so much so that he does not care if he is still alive when his goal is achieved. Despite this claim, in battles where he has felt his life is in danger, he often escapes, maintaining that as long as he can preserve his soul form, he will never die. According to Suguru Geto, unlike Gojo, Mahito can remain relatively calm and is the type to usually prepare his plans before acting. Despite his frivolous personality, he has shown on several occasions to be playful and cheerful; he even used to play alongside Hanami and Dagon in his free time. Kento Nanami describes him as extremely immature, with a childlike personality. If he feels that a human can serve his purposes, he will not hesitate to manipulate them until he slowly kills their soul. For example, when he befriended Junpei Yoshino, he saw him simply as a toy that would be fun to play with, however, when he realized that the young man would aid him in his plans to corrupt the soul of Yuji Itadori, he did not hesitate to use him. He forced the young man to become a curse user, fueling his hatred towards his own race. In the end, when Junpei became useless, he used him against Itadori and even killed him without a second thought.

Mahito, unlike other curses, has a human appearance. He has a thin and tall body, however, he is able to change the entire structure of his body and transform into whatever he desires. He received the nickname "Patchface" due to the number of scars he has on his face and throughout his body. His skin tone is light and somewhat pale. His hair is bluish-gray and long and is divided into three strands held with a band before reaching the end of his hair. His eyes are of different colors, the right one is represented as gray and the left one is blue. When he uses "Spirit Body of Instant Death," he is able to reveal his true form as a curse. His body structure changes completely: he loses his human face, his body becomes muscular, dagger-like blades sprout from his arms, he grows a long tail, and a series of twisted bangs or waves sprout from his head. Even in this form, he manages to maintain certain characteristics of his "patched" body.
