Born in Scotland, UK, John MacTavish was a lifelong football fan and often played as a goalkeeper.

One day, MacTavish was invited by his cousin, a member of the 23rd Special Air Service Regiment, to see what it was like to be in the British Army. Afterwards, MacTavish would often visit his cousin on weekends. At 16, he tried several times to join the SAS, and although he lied about his age, he was always caught.

After turning 18, MacTavish officially entered the recruitment process for the 22nd Regiment, an elite squadron specializing in covert reconnaissance, counter-terrorism, and hostage rescue. In 2014, during his training in Hereford, Captain John Price was MacTavish's evaluator. Recognizing his natural abilities, exceptional competence, and tireless dedication, Price became tough and strict with MacTavish to make him the best recruit. After learning that Hassan escaped to Las Almas with the help of the local cartel, Soap and Ghost joined up with Colonel Alejandro Vargas and his squad, Los Vaqueros. They scoured the town for Hassan. After being attacked by the Mexican Army, Soap, Ghost, and Alejandro lured them through mountain trails and down a river while the other Vaqueros escaped, with Shadow Company commander Phillip Graves arriving and supporting them soon after via gunship. The squad regrouped at a possible location of Hassan and assaulted the compound with the help of Shadow Company's gunship. They captured Hassan and made their way to another town, where they were again ambushed by the Mexican Army and the cartel. Shadow Company helped them survive until extraction.

Meeting The Nameless One Despite the successful capture, Task Force 141 is forced to release Hassan to avoid political fallout. However, they planted a tracker on Hassan's phone, allowing Price and Gaz to follow him. Meanwhile, Soap, Ghost, Alejandro, and Shadow Company obtained information about a party at a cartel estate where El Sin Nombre, leader of the Las Almas Cartel, might be located. Soap infiltrated the estate, claiming to have information. He was interrogated by hitmen Diego Salgado and Valeria Garza, and then told to wait upstairs, where he connected with an undercover Alejandro. They would later capture Valeria, after discovering she was El Sin Nombre, and take her to Special Forces headquarters.

Valeria informed Task Force 141 that the missile was located on an oil platform in the ocean, prompting them to infiltrate it with Graves and the Shadow Company troops to find the missile and its controls. Soap and Ghost inspected the platform, but the missile's controls were on the ship, prompting Soap and Graves to go to the ship by boat and advance to the bridge through the containers. They managed to clear the bridge, and Soap and Graves found the missile's control assembly just as it began to launch. However, following Graves' instructions, Soap managed to cause the missile to impact the oil platform, and the rig crew managed to escape just in time.

Betrayed Upon the team's return to Special Forces Headquarters, Graves announced a hostile takeover of the headquarters on Shepherd's orders, with the Vaqueros at the base arrested. A firefight broke out, in which Ghost and a wounded Soap escaped, while Alejandro was captured. Later, following Ghost's remote instructions, Soap recovered and managed to evade Shadow Company troops, before rejoining him and escaping to Alejandro's safe house. The two met with Alejandro's second-in-command, Rodolfo Parra, and discussed their plan to break into the prison.

After planning the release of Alejandro and the Cowboys, Soap, Ghost, and Rudy infiltrated the prison. They managed to free the Cowboys and escape with the help of Price and Gaz. The team was then informed by Laswell of the truth behind the Shadow Company's betrayal.

Arresting Graves and Hassan Task Force 141 and the Cowboys formed a new joint group, Joint Task Force - Ghost Team, and launched an assault on Special Forces Headquarters. Soap and Rodolfo eventually caught up with Graves, who commandeered a tank to kill them both. Using explosives and other equipment at the training ground, Soap managed to destroy the tank, believing Graves perished with it.

After the team retrieved Valeria, she revealed that Hassan was in Chicago with the last missile. Task Force 141 bid farewell to Alejandro and the Cowboys and headed to Chicago to stop Hassan once and for all. Soap, Price, and Gaz infiltrated the building where Hassan was being held, while Ghost kept an eye on them. The missile launched, much to their dismay, towards Washington. Eventually, the team caught up with Hassan, and Soap took the control box and hid from Hassan and the Al-Qatala forces. With Laswell's help, Soap stopped the missile, but Hassan grabbed him and nearly executed him before Ghost intervened by shooting him from afar.

Some time later, while celebrating at a bar, Laswell informed Task Force 141 that the Ultranationalists were behind the attack on Shadow Company that resulted in the theft of the missiles. Price quickly identified Makarov as their leader.
