Otoya is a handsome young man with short, straight white hair, a dark green streak in the middle and a dark green underside. His face is thin, with a prominent chin and dark, thin, low-lidded eyes. His slender limbs allow him to be agile on the field.

In Blue Lock, Otoya wears the official uniform (black with grey stripes) and tracksuit. During the Second Selection, he wore the number 33 jersey of the Red Team and, during the Third, the white number 33 jersey of the B Team. Only at the start of the Japan U-20 match did he wear the official blue uniform of Blue Lock, with the number 9 jersey.

Personality Otoya has a calm and easy-going personality, and rarely shows pressure or stress, even during a match. He has no qualms about provoking his opponents and seemed amused when Rin and Shidou started fighting after the match.

He is also very confident in his abilities, telling his team to defend during their Third Team match and not to respond. Despite this, he recognizes when a teammate or opponent impresses him, as seen when Chigiri's speed impressed him.
