Like all members of the Iwatobi High School Swim Club, Makoto has a female name. When he was in elementary and junior high school, he was invited to join the basketball club because of his good physique.[9] Despite being on the swim team, Makoto is afraid of the ocean.[3] However, in High☆Speed!, Makoto is not only afraid of the sea, but water in general. Makoto has been described as the wishy-washy type by Haruka because he is too nice and was unable to decide which club member would be his boyfriend if he were a girl.[10] Makoto and Haruka seem to have a game that consists of Makoto guessing what Haruka is thinking.[11] Makoto likes to eat green curry and chocolate.[8] Makoto's best subject is Literature, while he is bad at English and Art.[8] Makoto enjoys physical activity and manual labor.[8] Makoto was voted Sexiest Male Anime Character of 2013, beating Haru Yoshida from Tonari no Kaibutsu-kun with 24,861 votes.[12] Makoto won the Bodybuilding Contest at the revamped Iwatobi Swimming Club, where Gou was one of the judges. However, Gou gave the most points to Rin.[13] Makoto's voice actor, Tatsuhisa Suzuki, is the vocalist of OLDCODEX, the band that performed Free!'s opening theme, "Rage on". In Brazil he was dubbed by Renan Vidal.

Hiroko Utsumi, the director of the anime Free!, described Makoto as a "sweet boy." "He's your average, everyday high school boy. Strong, sweet, and a very helpful older brother. He was Haruka's childhood friend, so there's nothing he dislikes about him. On the other hand, he knows very well the things he likes about Haruka. And because of that, he knows how to deal with Haruka very well. Since Haruka isn't the type to talk much, Makoto accompanies Haruka and speaks in his place. If Haruka wasn't like this to begin with, Makoto would be so close to him. Since he's flawed, Makoto is there by his side. I wonder if Haruka would also notice if Makoto's great existence wasn't always about taking care of him," he states. "Actually, one aspect of Makoto's character is that he gets scared easily. He's big, but scary things come to him. It was a little sad when we decided to make him scared with this body style. My impression is that, because he's grown so much, he doesn't know how to use his size to his advantage. A smaller body really seems to fit that side of him," says the director.

Gou Matsuoka Makoto and Gou Matsuoka have a good, friendly relationship. Makoto was the first to notice Gou and the first to recognize that Gou is Rin's younger sister. From the day Makoto and Nagisa met Gou in front of Haruka's house, the three began hanging out a lot and spending time together at school. After talking with Makoto at the school pool, Gou decided to join the Iwatobi High School Swim Club. Unlike Gou and Nagisa's relationship, Gou never gets mad at Makoto, despite him calling her "Gou-chan."

Ikuya Kirishima Makoto and Ikuya first meet when Haru and Makoto are being drafted into the Iwatobi High School swim team. Unfortunately, Ikuya and Makoto don't have much of a relationship outside of the swim team because they're in different classes. However, they value each other's company and work well together. Makoto is the second to notice Ikuya's imitation of Haru (after Haru, of course). However, he dismisses it after asking Haru about it. When Ikuya finally tells the team about his loneliness and longing for his brother's company, Makoto consoles Ikuya by stating that no older brother can hate their brother. The two are probably the ones who face their feelings the most. Along with the rest of the team, they open up to each other, building more intimacy and affinity. Family Makoto has a great relationship with his parents; they love each other very much and talk a lot. His younger brothers, Ren and Ran, seem to admire and love Makoto very much as their older brother, and Makoto always takes great care of them both.

Haruka Nanase Haruka and Makoto have known each other since they were little, quickly bonding. They attended the same school in elementary and high school, and then moved in together when they went to university. Makoto often tries to discourage Haruka from doing stupid things, but his attempts are often unsuccessful. Makoto is Haruka's best friend; the two are very close and are always there when the other needs them.

Rin Matsuoka When Rin transfers to Iwatobi Elementary School, Makoto is the kindest and friendliest to him. The two, along with Nagisa and Haruka, are members of the Iwatobi Swimming Club and have competed in several relays together. After Haruka and Rin had their problems and Rin went on an exchange program in Australia, Makoto and he lost touch. When Rin returns from Australia, their relationship isn't the best, but when things start to work out, their friendship is rekindled. The two get along very well and are good friends.

Nagisa Hazuki Nagisa and Makoto are close friends, despite not having been in touch for a while. While both are generally outgoing and positive, Nagisa is lively and Makoto is kind and sensitive. They look out for each other. Makoto also knows when something is up; for example, at the squid festival, when Nagisa was acting paranoid and gave Makoto a wink, he knew to wait a moment without Haruka to ask Nagisa what was going on. Rei Ryugazaki Makoto and Rei have a good senpai-kohai relationship. Makoto tends to act like an older brother to Rei and often keeps him away from Nagisa's constant hyperactivity. Makoto was willing to risk his life to save Rei from the storm.

Makoto is a nice and caring guy who is always concerned about his friends' well-being. Due to his caring personality, he is often seen with a gentle smile on his face. However, he is quite weak-hearted and can get scared easily,[2][3] as shown when he appeared scared of the possibility that his old Swim Club might be haunted.[2] When he gets scared, he tends to hide behind Haruka or cling to him in some way.[2][3]

Makoto also seems to be one of the more level-headed people in the group and can get a little tired of his friends' antics and peculiar mannerisms.[2][4] He is the only one who usually restrains Haruka for stripping naked or for randomly diving into the fish tank.[2][5][4]

Makoto and Haruka have been together for so long that Makoto is able to read Haru's mind.[2][4][6] He is also shown to be somewhat manipulative, often using Haruka's love of water to his advantage to convince Haruka to do what he wants.[2] It has also been shown that Haruka dislikes it when people use his first name, but Makoto always somehow manages to win the argument and calls him "Haru".

Makoto is a tall young man with short olive-green hair and droopy green eyes. His usual school uniform consists of light brown pants, a white shirt with a green tie, a dark gray unbuttoned blazer, and orange sneakers. During the spring and summer, he also wears the summer uniform, which consists of gray pants, a white shirt with a dark gray collar, a green dotted tie, and orange sneakers.

When swimming, he wears long black swimming shorts with green patterns on them. He is seen wearing black-framed goggles once, but it is not stated whether he actually needs them or not.
