In the present, Sukuna was partially revived when Yuji Itadori ingested one of those fingers, becoming its vessel. When Sukuna manifests in Yuji Itadori's body, his appearance changes significantly, though he retains the base of Yuji's body. Here's what he looks like:

Face: Two black lines (like tattoos or marks) appear running from the corners of his mouth to his cheeks. Additionally, a black line runs vertically down the center of his nose and another across his forehead, giving him a more intimidating and demonic appearance.

Eyes: When in control, Sukuna's eyes become sharper and slanted, with thin, red pupils, giving him a fierce and ruthless expression. Occasionally, two extra eyes even appear on his face (in the cheek area), bringing the total to four, further enhancing his inhuman appearance.

Expression and Posture: His face often displays a sarcastic or cruel smile, with a confident, mocking, or downright sadistic attitude. His body language changes, adopting a more relaxed yet threatening posture, as if he were always certain that no one could defeat him.

Voice and Attitude: Although physically still Yuji, Sukuna has a deeper, colder, and more authoritarian voice. His tone reflects superiority and contempt toward others, including Yuji himself.

In short, Sukuna in Yuji's body retains his basic physical features, but the black markings, extra eyes, and malicious energy make him completely different and much more terrifying.

He is brutal in combat, using cursed energy techniques such as his Domain Expansion: Malevolent Sanctuary, an area in which he can cut any target within range with surgical precision. As Yuji consumes more fingers, Sukuna's power grows, posing a threat to the entire jujutsu world.

Despite being feared by all, Sukuna holds many mysteries, including his true purpose and exact origin. His presence in the story adds constant tension, as his complete resurgence could mean the end of humanity.
