He is a man of average height, with fair skin and gray eyes. He has a sharp gaze and no eyebrows; his hair is a warm gray, kept short. His bangs are parted in two thick strands across his forehead, one in the center and the other on the left side of his head. One of Sanemi's most prominent features is his marked physique and the numerous scars that crisscross his face, chest, and arms, which he doesn't hide and keeps as a reminder of how hard his profession is, a profession he is proud of.

Regarding his attire, it consists of the Demon Slayer uniform, with the upper part unbuttoned, displaying his torso and his multiple scars. He also has a white jacket, and keeps his Nichirin sword on his belt. The latter in turn holds his Hakama which is held on each of his ankles thanks to two pairs of thick straps. Sanemi was the eldest of seven children born into a dysfunctional home, with an extremely abusive father towards him, his mother and his seven siblings (Genya included) whom he beat continuously and for no reason, until one day, someone who held a grudge against his father stabbed him to death. When Genya goes out into the street to look for a doctor for his seriously injured siblings, he finds Sanemi with his mother dead at his feet. Panicking, and unaware that the "wolf" that attacked them was her mother transformed into a Demon, Genya screams and accuses Sanemi of having killed her.[4] Sanemi, seeing this, goes into shock, suffering from Achromatopsia while her mother's body begins to disintegrate upon contact with the first rays of the sun.

After that tragic event, Sanemi abandons his brother and sets out to hunt Demons on his own, suicidally. Without knowledge of Demon Slayers or Nichirin Swords, he instead armed himself with a multitude of weapons and fought against the Demons.
