Since childhood, Sae has been cold, direct, and serious. He has only been interested in becoming the best midfielder/forward in the world and has only had time for what brings him closer to his goal. Sae can also be arrogant and condescending, looking down on others even if they are older than him and have greater organizational authority. He is selfish in his football game, but has proven capable of controlling himself and playing his position without trying to overshadow others unnecessarily.

Sae takes great pride in being a footballer and looks down on Japanese football and all its players. He claims he would rather die or play in Europe with a group of college kids than play in the J-League or the Japanese national team. He is very confident in his abilities and firmly believes that no one in Japan is worthy of his skills as a teammate. He dislikes being born in Japan, and says things as if he were simply born in the wrong country.

Sae seems to easily command the respect of most people in a room thanks to his reputation and ability, even with his bad attitude and blunt way of speaking. Furthermore, he can manipulate and convince loud and rude people to listen to him or do things his way. Although Sae is a difficult person, he is not impossible to work with, especially if he gets something in return. Out of pure self-interest, he decides to stay in Japan on a whim after learning about the Blue Lock Project and even joins the Japanese U-20 national team for the match against Blue Lock Eleven. But when his interest is satisfied, he has no qualms about abandoning those he deems unworthy of his time.

Sae is a tall, slender young man with magenta hair and fine blue-green eyes framed by long eyelashes, much like his brother, Rin. During the national representative team's matches, he wears the white number 10 jersey of the Japan U-20 team with a black undershirt and black gloves. Although he may also wear other types of clothing at times, nothing too formal.

Sae Itoshi (Itoshi Sae) is a football prodigy, recognized as the best player in Japan. Sae is also a member of the New Generation World XI and the Re Al youth team. He is the older brother of Rin Itoshi.

Sae's main goal before the start of the series was to become the best striker in the world, and in turn, the best player in the world, but after leaving Japan and experiencing the professional league in Spain, he changed his dream to becoming the best midfielder in the world after realizing how truly weak Japanese football is.

At the beginning of the series, he was in Japan only to renew his passport, but after learning about Blue Lock, he decided to stay. He subsequently joined the Japanese U-20 national team to face Blue Lock Eleven in the U-20 match. After the match, he returned to Spain, but joined the newly formed Japanese U-20 national team in the U-20 World Cup so he could challenge Spain and beat Bunny Iglesias.
