However, this procedure comes at an emotional cost: the person will lose all loving feelings toward their loved one. Although it eliminates the symptoms, surgery is also a way to renounce that love forever. Alternatively, if the love is truly reciprocated, the symptoms disappear naturally.

However, this dandy and playful aspect hides a much darker side to him, one with high levels of self-importance, and will not hesitate to use physical violence when others do not act according to his very particular values or expectations. He is known to be narcissistic, and claims that his love for himself cannot be compared to anyone else's, and does not see many people who are on his level. Alastor is described as a man of duality. He highly values good manners, affability, and intelligence in others, and will look down on those who do not meet his standards, often playing by these arbitrary rules in regards to himself and his own conduct. He has a strange sense of morality, which is described as "abnormal", and has been noted to be quite sadistic, even cannibalistic, devouring lesser demons or those who have incurred his wrath. Despite this, he maintains a close friendship with the other cannibals of Hell, including the inhabitants of Cannibal Town. His smile is a very self-imposed form of ego and a display of power and dominance; he looks down on anyone who lets their true emotions show, and even when facing a rival in strength, if they let out a scowl, Alastor will view them as truly weak.
