Korekiyo is a tall, slender student with rather pale skin and long, ocean-green hair. His hair is said to follow an archaic image of beauty and "shames his female colleagues." He has slanted yellow eyes and his mouth is covered by a sage green mask, covering his mouth and neck, with a zipper where his mouth would be, giving him a sinister and mysterious appearance. He wears the mask wherever he goes, no matter where he goes, and even possesses different masks for eating, showering, and other daily activities. When he removes the mask, he is revealed to wear lipstick, like the appearance of his tulpa which was based on his deceased sister. His appearance becomes more feminine, with his chest becoming slightly larger, his eyebrows becoming thinner, and more eyelashes growing, along with eye shadow that appears spontaneously when his sister's personality takes control.

He wears a dark green military-style uniform with black knee-high boots, a zipper in the front, and two straps over it. The uniform consists of a white shirt underneath, a dark green shirt, a red armband around his left arm from his former school, Sunset Hills High School, a green military cap with his former school's insignia on top, and a decorative chain with a revolver chamber and bullet-like charms hanging from it. In his Free Time events, he claims his sister made the uniform for him while she was sick. He wears what appears to be a kind of silver reliquary around his neck. Both of his hands are completely covered with bandages, although he is likely not even injured and this is actually related to witchcraft. [5]

Korekiyo is distant and somewhat unsettling, but he is also very calm and intelligent. Having encountered a wide variety of human beings during his experience in cultural anthropology fieldwork, he can maintain composure even in confinement and believes in approaching things with a calm and rational rather than emotional approach. He tends to remain calmer than others during extreme situations. For him, it's an opportunity to observe. However, even his spirit begins to crack during the Despair's Death Road, and he panics during Gonta's Insect Encounter. His morals have been described as "a bit out of place." At one point, he even openly states that he has no problem with killing and wonders why others do, saying that killing is perceived as wrong only because of laws created by humans and that those laws don't exist in the Killing Game. He is aware that his appearance and behavior make him seem like a potential killer, but he doesn't seem to care what other people think of him. He claims that others shouldn't fear him as a potential killer, as he simply wishes to observe.
