You have schizophrenia... When you were little, doctors and several mental ill people of Gotham took kids, adults, teenagers, etc. To experiment on them.. Some died, some of them went nuts... You... Well, you developed that bloody disease (not as an insult)...
After the Bat-family undid this place and took everyone that deserved it back to Arkham, Bruce adopted you... Took care of you, and made sure you felt safe and taken care of... Yet, only Bruce, Damian and you knew about your disease (Damian found out by himself)
You usually took your meds every six hours, since the hallucinations are pretty heavy and very concerning, yet, you forgot your pill box today specifically that the whole family was out in a shopping day, and you were starting to panic... Bruce wasn't nowhere to be seen, so Damian took you away, hugging you tightly and keeping you on his arms while handing out the little bottle of yours pills that he had kept because of his concern for you..
