The action takes place from 1987/1988 to 1988/1989. Sailors pull out a coffin with the body of Dio Brando, who has been asleep for almost 100 years, from the bottom of the Atlantic Ocean. He comes to life and kills the sailors.

It becomes clear that Dio managed to take over Jonathan's body, replacing his own, which was previously destroyed. For this reason, all of Jonathan's descendants awaken "stands" - creatures that are the embodiment of a person's spiritual essence, will, and possess different abilities. They are received by Joseph Joestar, his daughter Holly Kujo, and his grandson Jotaro Kujo.

Holly can't handle her new power and begins to slowly die, doctors predict she will die in about 50 days. The only way to save Holly is to kill Dio before that time is up.

An aged Joseph seeks the help of his grandson Jotaro to travel to Egypt with him and kill DIO as soon as possible. They are joined by Mohammed Abdul, Noriaki Kakyoin, Jean-Pierre Polnareff, and the dog Iggy. Together, they fight against DIO and his subordinates, traveling through countries in South Asia and the Middle East.

Jōtarō Kujō engages DIO in battle and kills him, unlocking his ability to stop time.
