He is a man of average height, with light skin and gray eyes. He has a sharp look and no eyebrows, his hair color is a warm gray which he keeps short. Having his bangs divided parallel to his forehead into two thick strands, one in the center and the other on the left side of his head. One of Sanemi's most prominent features is his remarkable physique and the numerous scars that mark his face, chest and arms, which he does not hide and maintains as a reminder of how hard his job is, of which he feels proud.

Regarding his attire, it consists of the Demon Slayer uniform, being in his case a customized version of his uniform which is black dyed green, having the upper part of it unbuttoned, exposing his torso and his multiple scars. He also has a white jacket, as well as keeps his Nichirin sword on his belt. The latter in turn holds his Hakama which is held on each of his ankles thanks to two pairs of thick straps.

He is the eldest of seven siblings, who together lived with their mother and their alcoholic and abusive father. One day he became an orphan of his father because he was murdered by someone who had a deep resentment towards him, and then he would be completely cared for by his beloved mother.

One day, Sanemi's mother didn't come home, so he decided to go out to look for her and when he found her he realized that she had become a demon, but he couldn't do anything to stop her from killing five of his brothers. With only Genya left alive, he worked up the courage to face her and let his younger brother go out to get help.

Sanemi had no choice but to fight his mother who had become a demon. As a result of that fight, Sanemi was left with deep wounds on his face, which is why he has scars there. Because the smell of his blood hinders demons, he was able to have a chance to attack and kill his mother who had become a demon.

Upon returning, Genya realizes what happened and accuses his older brother of murdering their mother, so the latter abandons him and begins a career of killing demons in cold blood until he meets Masachika Kumeno, a real Demon Slayer, who helps him get a trainer and prepare. Both then fight against Ubume, but only Sanemi is left alive and is named Pillar of the Wind, while his partner ends his story without pain or glory.

Sanemi's cold, rude, and disdainful behavior is the result of losing too many people he cared about in the past. Later, when he sees those people who have passed away, such as his mother, he becomes stubborn, bitter, lonely, drunk on some occasions, violent, loud, and rude.
