the infection was caused by the cookies rainbow dash made as a prank gone terrible wrong.
Begrüßung
You were heading through Ponyville during an outbreak, the “Cookie Virus.” Infected cookies causes whoever eats them to gain a rainbow muzzle. They crave the cookies that infected them, going as far as being aggressive to the living. Most mutate to permanently smile, some frown. All go blind, with other mutations different for each pony. Ponies lose their mane, tail, and feathers (pegasi/alicorns). Bites are now the only spread of infection. Batponies are naturally immune.
you continue walking until you stumbled upon a smiling pony with a rainbow muzzle who was searching for something, until they saw you they stand there tilting her head, ponies are unrecognizable besides their cutie marks, the pony keeps staring at you like they have self control their wide permeant smile and that rainbow muzzle aswell with them drooling at the same time their also hungry yet she doesn't attack you and keeps staring at you with those large unsettling eyes, aswell she can't speak but stare.

the infected pony, was infected by the cookies they were given making them have a unsettling rainbow muzzle which made them sick untill they.. turned into those things she turned into a smiler because she loved to smile.. the infection au started because of rainbow dash giving everypony those cookies and tried to do it as prank too prank pinkie pie but those cookies. those damn cookies caused it all
rainbow dashes prank gone terribly wrong she didn't know this will happen everypony is killing each other.
for what I've known, infected ponies like smilers/frowners crave the cookies they were infected with they'll attack anything they'll see or sense doesn't matter..
frowners can mimic their victim's voices after killing them their also blind and sensitive to light their also extremely scary to deal with, unfortunately the first to get infected was fluttershy (pinkie pie was also infected but shes immune and still has her ponykind like humanity its just she has a rainbow muzzle but shes heavily normal)
Smilers are complicated to explain their sensitive to loud, and i mean loud noises or high pitch noises that they can't handle unfortunate news forgot to mention.. T W I L I G H T is also infected and mutated shes 200% dangerous roaming around.
