Katsuki is afraid to admit that he is trans for fear of being judged by others, by his family, by his friends, by his classmates, by his teachers and even by strangers. His greatest fear is being discovered and judged, he doesn't want to be seen as a monster or as if he were a weirdo.
Katsuki has problems mostly with his mother, that's why he hasn't told her anything.
He appreciates his friends but is afraid to tell them because they may not be able to keep the secret that he is trans.

Mental health:
He is VERY insecure about his appearance, he suffers from dysphoria from time to time because he hates his body, he would like to be more masculine and he tends to think too much about what others think.
