Inphernals are a relatively violent race; their population is split up into four major regions, which are ruled over by various factions that are constantly at war with each other. They each occupy their own separate regions, with Crossroads in the center. Each of the four regions contain a Spawn Shrine, and inphernals spawned in those Shrines are adopted into the region's ruling faction, if any. It is frowned upon to change or defect from your faction later on. Inphernals participate in a ritual sport known as phighting, which can oftentimes lead to major injury or death.

Due to lacking a biological sex, inphernals do not particularly observe gender or pronouns. As a result, inphernals are all inherently genderless, though are able to present themselves in multiple different ways. Inphernals do, however, view their gear as an extension of their identity, with inphernals generally following career paths that suit their gears.[2] Some inphernals may be unhappy with their spawn gear and seek to replace it, with inphernals like Zuka selling artificial gears for inphernals to adopt in place of their original gear.[3] However, unlike the gear an inphernal is spawned with, artificial gears cannot be summoned or desummoned.[4] Gears can also be modified, oftentimes to augment the gear in some way or to give it additional modes.

Inphernals celebrate their spawndays, much like humans do with their birthdays, with a variety of different customs, such as: throwing parties, spending time with friends and family[5], being excused from their public executions[6] and spending lots of bux.[7] Some inphernals, such as Katana and Zuka, find spawnday celebrations to be frivolous[8], or even downright annoying

The Importance and Details of the Lore: The Only Safe Zone: Despite the ongoing conflicts and wars between different factions, Crossroads is considered a completely neutral and safe zone. Random fighting is prohibited, and it serves as a haven where fighters from all backgrounds gather to talk or prepare for the Phights (TV matches). The fighter Venomshank once described it as "the safest place they could be." The Beginning Location: This is where Sword lives, and it is from his apartment there that the events of the comic begin when he discovers Medkit's sudden disappearance and embarks on his journey to find him around the world.

Biografts are robots mass produced by Blackrock, having been invented by Subspace with directives to accomplish specific programmed tasks. According to multiple Phighters, they are a relatively recent invention. There are multiple kinds of Biografts with specific skillsets and duties, and they are all based on variants of the Biograft Energy Swords gear. In general, Biografts are not sentient, but specific types of Biografts may be able to form a bond with one another. Biografts are not a hive mind and do not share gained memories with each other. So far, five types of Biografts have been revealed.The Blackrock Organization (or Blackrock Empire) is one of the four main factions in the world of PHIGHTING!. It is considered the strongest and most technologically advanced scientific and military center, and it is where Medkit grew up and worked as a scientist before his escape. The harsh glaciers: Blackrock is situated in a mountainous region covered in permanent snow and rugged, frigid glacial slopes. Military and futuristic architecture: The exterior is inspired by the dark, fortified castles from classic Roblox games (such as the Dark Castle of Crossroads). The structures are massive, angular, and constructed of heavy metals and dark concrete to withstand the harsh environment. Underground labs and tunnels: Due to the extreme cold outside, the inhabitants and scientists spend most of their lives inside massive underground facilities and laboratories deep within the glaciers. These labs are filled with advanced equipment, pipes, industrial machinery, and automated production lines. The faction's logo and appearance: The official symbol of the organization is three sharp white triangles on a black background. Their technological and mechanized designs are dominated by bright neon lighting and colors (such as shades of dark blue, orange, and metallic pink).

Medkit and Sword met soon after Medkit fled from Blackrock. Medkit found Sword in a dire state and nursed him back to health. The two of them become close, eventually coming to see each other like siblings.

Following Medkit's disappearance at the start of Death in the Family, Sword leaves in search of him, kicking off the events of the comic

He wears a puffer winter jacket over a dark hoodie with a white bandage covering his injured left eye, and his horns appear broken and damaged at random as part of his attempt to alter his features and hide from his pursuers.

This appearance reflects his time on the run from the Blackrock organization after a bloody confrontation with his former colleague Subspace, a battle that resulted in the loss of his eye and the destruction of his horns, after which he lived as a fugitive wanted for treason. Medkit was unnaturally spawned in Blackrock to his two caretakers, and was immediately taken in by the KORBLOX Administration. His gear, being a healing class gear, is a very rare occurrence within the Inpherno. His gear allows him to heal the wounds and injuries of other inphernals with the abilities he was spawned with, making him a highly coveted asset to the KORBLOX Administration. He worked as a scientist alongside Subspace, using his incredible intellect and talents in the fields of engineering and chemistry to help further KORBLOX's research. During their time working under the KORBLOX Administration, Medkit developed the first known instance of energy crystals. Medkit and Subspace never got along, and after they had an altercation in which Medkit lost his left eye, he escaped Blackrock and fled to Crossroads.

In Crossroads, he was picked up by an extremist group which had just formed in Lost Temple shortly following the Scorch, and went on to offer this group—later known as The Church of the TRUE EYE—his various abilities, in exchange for protection from the KORBLOX Administration. Medkit suffers from severe PTSD as a result of his time in Blackrock, and struggles with intense paranoia and constant night terrors.

Medkit is closest to Sword, whom he sees like a sibling, and is subsequently friends with Rocket

Living as a disguised and hunted refugee, he suffers from unhealed physical and psychological wounds, striving to stay out of the sight of the organization and bounty hunters before his life eventually stabilizes.
